Individualized support for adults with intellectual and developmental disabilities to live independently in the home of their choice — on their terms.

What Is Supported Living?

Vitalia Care Services provides Supported Living services to adults with intellectual and developmental disabilities who choose to live in their own home or apartment in the community. Our role is to provide the individualized supports each person needs to live as independently as possible — on their own terms, in a setting they choose.

Supported Living is not a group home. The individual lives in their own space. We come to them.

What We Provide

Vitalia’s Supported Living team assists with:

  • Daily living skills and personal care
  • Health and medication support
  • Community access and participation
  • Financial management assistance (when authorized by ISP)
  • Behavioral support consistent with each individual’s Positive Behavior Support Plan
  • Crisis prevention and emergency planning
  • Coordination with case managers, medical providers, and ISP teams

All supports are person-centered and based on each individual’s ISP. We do not provide a one-size-fits-all program.

Who Is This For

Adults (18 and older) with intellectual or developmental disabilities who:

  • Want to live in their own home or apartment in the community
  • Are eligible for Oregon ODDS developmental disabilities services
  • Receive Medicaid through OSIPM or Healthier Oregon
  • Have an Individual Support Plan (ISP) that identifies a need for supported living services

Case managers and CDDPs in Washington County and surrounding areas can refer directly.

How Referrals Work

Case manager initiates contact

Call or email us. We confirm availability and capacity to discuss the individual’s needs.

ISP & documentation review

We review the individual’s ISP, support documentation, and eligibility. We coordinate with the case manager throughout.

Service agreement & planning

We develop an individualized implementation strategy with the individual and their ISP team before services begin.

Services begin

A dedicated Direct Support Professional is assigned and services begin in the individual’s home or preferred setting.

Our Team

Vitalia staff speak English, Spanish, Amharic, and Swahili. For individuals whose primary language is not English, we actively recruit staff who can communicate in the individual’s language and engage translation services when needed. Cultural competency is not optional — it is built into how we hire and train every Direct Support Professional.

Regulatory Authority

Vitalia Care Services is certified and endorsed by the Oregon Department of Human Services, Office of Developmental Disabilities Services (ODDS) to provide Supported Living services under Oregon rules.
