How Oregon’s I/DD system works.
Understanding the pathway from eligibility to services helps families and individuals know where they stand — and who to contact at each stage.
Contact Washington County CDDP to apply. An eligibility specialist uses the state’s eligibility criteria to make a determination.
Once eligible, a service coordinator completes the Oregon Needs Assessment to identify support levels and service needs.
An ISP team (individual, family, coordinator, providers) creates the plan identifying services, goals, and authorized hours.
The individual or family chooses a provider for each ISP-authorized service. Providers like Vitalia are then contacted for referrals.
Following prior authorization from ODDS and the entry process, services start. The ISP is reviewed and updated annually.
Eligibility applications are processed by the Washington County Developmental Disabilities program (the local CDDP). You can apply by calling 503-846-3150 or visiting 5240 NE Elam Young Parkway, Suite 150, Hillsboro, OR 97124. Applications are available in English and Spanish. There is no cost to apply.

Privacy & dignity — the right to privacy in your room, space, and personal matters.
Community access — the right to participate in community activities and come and go as you choose.
Visitors & family — the right to have visitors at times you choose without restrictions.
Non-discrimination — services cannot be withheld based on race, language, religion, or national origin.
