Educational coordination — school team collaboration, IEP support
ISP team collaboration — full participation in every child’s Individual Support Plan
Community integration — recreation, access, and participation as HCBS rights

Who Is This For

Children (under 18) with confirmed intellectual and/or developmental disability who:

  • Are eligible for Oregon ODDS developmental disabilities services
  • Receive Medicaid through OSIPM or Healthier Oregon
  • Have an Individual Support Plan (ISP) that identifies a need for 24-hour residential placement
  • Are referred through the Washington County CDDP or an authorized case management entity

Note for families: Placement is accessed through your child’s assigned case manager. Families are always welcome to call us with questions at any point before, during, or after that process. We welcome the conversation.

Our Team

Our Direct Support Professionals are trained to each child’s specific needs, behavioral support plan, and medical requirements before their first shift. Staff speak English, Spanish, Amharic, and Swahili — enabling natural communication for families whose primary language is not English.
